Abstract: The goal of this project is to advise both partners of potential benefits and risks of a model/s of care where residential and care and support at home services and their workforce have been combined.We are seeking detailed case studies of instances (in and outside of Wales) where:- regulated provider(s) deliver both residential and care and support (either in an accommodation based setting or not) services within a specific locality.- regulated provider(s) and / or commissioner(s) have brought together a workforce from a residential setting (with / without nursing) alongside care and support at home workforce for the purpose of management, developing support networks, learning, and training.- provider(s) and /or commissioner(s) have brought the above workforce together and deployed their skills and capacity interchangeably to:~ deliver care and support and /or health related tasks in both the care home and the community.~ extend the range of early intervention and preventative support services offered in the community e.g. short breaks for carers, day care services, medicine management support, 24 hour falls response service, night-time or palliative care support.... - Documents :
