Description : Bristol, North Somerset & South Gloucestershire CCG (the CCG), on behalf of the Healthier Together partners, is seeking expressions of interest from organisations to provide an End of Life Shared Care Plan solution across Bristol, North Somerset & South Gloucestershire (BNSSG).Work completed for the first Bristol North Somerset South Gloucestershire (BNSSG) Electronic Palliative Care Co-ordination System (EPaCCs) provided the opportunity for many clinicians to view a patient's end of life requirements on a shared care plan. However, with limited access, numerous data fields to complete and sub-optimal updating processes, an improved solution has been prioritised within Healthier Together's LTP and is now funded as a priority.The requirement for a more user-friendly shared care plan has been exacerbated by Covid-19, where the limitation of the current solution has led to many not having their end of life wishes met.
