Description : The Council wishes to establish a contract for the provision of Home Support for Adults in Prison at HMP Birmingham. There is a requirement for the delivery of care and support for service users in HMP Birmingham. The contract will support service users to achieve their identified outcomes in order to improve their independence and ability to self-manage and to achieve and maintain their desired potential in relation to their physical, intellectual, emotional and social capacity. The Provider shall ensure that by providing the Service it achieves the aims and objectives of the Service: o to enhance the quality of life for people in Prisons with unmet, eligible adult social care needs; o delaying and reducing the need for Care and support in association with appropriate agencies; o ensuring people in Prisons have a positive experience of Care; o Safeguarding adults whose circumstances make them vulnerable and protecting them from avoidable harm (as set out in The Care Act); o to improve and maintain individuals' health and wellbeing through increased independence, choice, control, dign... - Documents :
