Description : NWL CCG requires a community improvement service to enhance access to health services for people with a learning disability and autism (LD&A). The aim of the service is to improve access to health services and to ensure that medical care is received more swiftly by working directly with people with LD&A and their carers, families and professionals, as well as engaging with local community services and providers. The aim is to ensure the system hears the voices of people with a LD&A and their families and carers directly.Key services required include:- Knowledge of the services that are available.- Knowledge of pathways to access available services.- Annual Health Checks.- Health Passports.- Dental checks.- Trained staff with an understanding of LD&A and how to support and make reasonable adjustments.- Accessible information (Easy to read format and information for non-English speaking individuals).The CCG currently has a contract in place with Community Connex (Harrow Mencap) to provide the above services.
