Description : NHS Brighton and Hove CCG, NHS West Sussex CCG and NHS East Sussex CCG (the NHS Sussex Commissioners) are commencing market engagement activities for a specialist Medicines Optimisation in Care Homes (MOCH) Service as part of the Sussex Health and Care Partnership (SHCP) Enhanced Health in Care Homes (EHCH) programme.The SHCP EHCH programme developed and agreed a best practice Multidisciplinary Team (MDT) model which includes the MOCH element.
