Description : Hammersmith & Fulham Council are tendering for a service to provide a sexual health promotion, prevention and support for Hammersmith and Fulham residents.The provider will need to:- deliver universal and targeted campaigns and literature aimed at reducing the transmission and acquisition of sexually transmitted infections (STIs)- deliver mixed universal and targeted support services aimed at reducing inequalities and improving sexual health outcomes- provide support and peer support groups to residents to address their needs for example chemsex or self-management for people living with HIV The following core elements will need to be included as part of the service offer:- promote good sexual health to enable residents to make informed decisions - provide an inclusive and equitable service which delivers tailored prevention and support services to all the diverse communities resident in the borough- work within a whole system approach to sexual health and monitoring service user and population outcomes as defined by the Office for Health Improvement and Disparities (OHID) - provide targeted services raising awareness of HIV and the prevention of its onward transmission - work with GPs and other specialist sexual health services to facilitate access to sexual health screening ... - Documents :
