Description : NHS Arden & Greater East Midlands Commissioning Support Unit (AGCSU) on behalf of the Commissioner(s), NHS Derby & Derbyshire CCG (CCG), wish to commission a Continuing Healthcare Assessment, Review, Case Management and Brokerage Service and invite interested providers to submit their expression of interest for this service.The successful service provider will be responsible for delivering an 'end to end' service in terms of assessment, case management and review of patients funded by the CCG to ensure patient safety, effectiveness and both clinical and financial benefit. The service will include negotiation and financial oversight of Personal Health Budgets (PHBs) and implementation and delivery of an effective and efficient Brokerage Service.The service is a Schedule 3 Service and is being procured under the Light Touch Regime (LTR) of the Public Contract Regulations (PCR) 2015.
