Description:
The Tier 3 service across Norfolk and Waveney will offer the opportunity for service users who meet the qualifying criteria to access comprehensive support via a weight management programme tailored to their needs including reasonable adjustments in line with NICE Guidance. The Tier 3 weight management service will incorporate both standard services and enhanced services for those service users with more complex health needs (co-morbidities) either independently or in partnership with other providers.The service aims to:• Encourage long term behaviour change by offering a programme which promotes physical activity; promotes healthy eating and supports the psychological barriers to unhealthy relationships with food; supports and recognises the relationship between mental health and obesity and offers a system throughout the programme which will support the psychological needs of everyone who attends.• Reduce the number of people living with obesity in Norfolk & Waveney, thereby improving long-term health, reducing the burden of disease, and decreasing the need for social care, while also preventing or reducing the incidence and healthcare activity related to obesity-related illnesses.• Function as part of a seamless care pathway for adults with obesity; to proactively work in partnership with services provided at tiers 1, 2 and 4, as well as other existing services supporting behaviour change• Undertake comprehensive stakeholder engagement to inform practice, referral confiden...
