Description : Surrey County Council is looking to procure a number of Collaborative Reablement Providers for their new Collaborative Reablement Service. The Service can be briefly described as one where providers deliver a reablement service in partnership with Surrey County Council's Reablement Teams. Surrey County Council's Reablement Service can be described as a short term service delivered within the clients own home for up to 6 weeks focusing on re learning or developing new skills, regaining confidence and utilising personal strengths to maximise independence. The Service responds to referrals from both the hospitals and also from the community teams - endeavouring to work preventatively, avoid unnecessary admissions and enable clients to remain safe and independent at home for longer. Reablement forms one of the tertiary care services, alongside short-term crisis response services, intermediate care services delivered by health professionals and bed-based intermediate care services. It is a short term (up to six weeks) intervention following hospital discharge or after an emergency at home, the aim of which is to help people to learn to accommodate and manage their illness or condition and maximise their level of independence by learning or relearnin...
